Need to have custom report to filer computer group membership

 unable to collect the Computer Objects “Member of” Group attribute. I need to have this field for our report. Can you please build me the custom report to have below parameters.

1. Domain for computer object

2. OU for computer object

3. Group membership for the computer object

4. computer object last logon time stamp

5. computer object last password change

    Domain Name (Domain)
    Canonical Name (Domain OU) 
    Computer Name (Domain Computer) 
    Description (Domain Computer) 
    Distinguished Name (Domain Computer) 
    Password Last Set (Domain Computer) 
    Last Logon Timestamp (Domain Computer) 
    SAM-Account-Name (extended) (Domain Computer)
