CTRL-SHIFT-ALT-F does not work for Refactor

Try re-installing the plug in first. If this does not works, go to Resources, Send Logs and select View Log files. Post the log file here. 

I see in the logs that you are running ApexSQL Refactor 2017.05.0283 which is out of date. Can you update from ApexSQL Menu, ApexSQL Refactor, Resources and then Get Updates.

About dialog box says: 2020.03.0414

That's odd, the log files sent yesterday had date from 2018. Any chance you can generate new log files after trying the keys combination a couple of times?
